Tax Document Checklist

Having all your documents for our appointment will help us best utilize our time together. Here are some common items that apply to many taxpayers that you can use as a guideline. This is not a complete list and those who are self-employed or own rental properties will have additional items.

Tax Organizers

Income

  • W2’s

  • Interest Income on Form 1099-INT

  • Miscellaneous Income on Form 1099-MISC or 1099-NEC

  • Dividend statement 1099-DIV

  • IRA/Pension/Annuity/Retirement distributions on Form 1099-R

  • Social Security Form SSA-1099

  • Gambling earnings on Form W2G

  • Capital Gains statement from your brokerage firm Form 1099-B

  • Schedules K1 from s-corporation, partnerships, or trusts

  • Alimony received if divorce was prior to 1/1/2019

  • If you sold your home or any other home that you owned Form 1099-S

  • Any other additional income

  • If you are self-employed or independent contractor, you may have additional items

  • If you own rental property, I’ll need the rents received and rental expenses

  • If you have distributions from a Health Savings Account Form 1099-SA

Deductions and Credits

  • Home mortgage interest

  • Home mortgage insurance premiums

  • Property taxes paid

  • Charitable contributions

  • Contributions to an Individual Retirement Account (not employer sponsored)

  • Contributions to a Health Savings Account

  • If self employed

    • Did you contribute to a SEP or SIMPLE?

    • Amount of your self-employed health insurance premiums paid

  • Alimony paid if divorce was prior to 1/1/2019

  • Loan interest on student loans

  • Statement(s) from care providers if dependents were in an aftercare program

  • If you or your dependents attended college, Form 1098-T from college/university/institution

  • Form 1095-A if you had your health insurance from the Healthcare Marketplace. This comes from www.healthcare.gov

  • Did you install any improvements to your home that meet Energy Star requirements (i.e. solar panels, exterior doors, windows, insulation)?

  • Dates and amounts for any estimated taxes paid.
